WOOA Plastic Surgery › Frequently asked questions
When should breast revision surgery be considered?
The WOOA website says breast revision is considered when an implant is damaged, when inflammation, bleeding or capsular contracture causes shape change or pain, when asymmetry develops, or when you are unhappy with size or shape. Capsular contracture is graded in four stages: revision is not recommended for grades 1–2, while grades 3–4 may need revision following diagnosis. Revision takes about 2 hours under general anesthesia, and the fee schedule lists it at KRW 10M–33M (VAT not included; varies with consultation and condition).

When revision is considered (per the clinic)
- When an implant has a problem or is damaged
- When inflammation, bleeding or capsular contracture causes shape change or pain
- When breast asymmetry has developed
- When you are unhappy with breast size or shape
- When implants were placed 10 or more years ago and may have deteriorated, or Allergan textured (Bellagel) implants were used (implant replacement page)
Capsular contracture grades (per the clinic)
| Grade | Condition | Revision |
|---|---|---|
| Grade 1 | Feels soft, no discomfort | Not recommended |
| Grade 2 | Shape unchanged but feels slightly artificial | Not recommended (steroid injections or ultrasound treatment to loosen the capsule are described) |
| Grade 3 | Implant is palpable and the appearance is deformed | May be needed following diagnosis |
| Grade 4 | Clear deformity and pain | May be needed following diagnosis |
Revision methods compared
| Method | When it is used (per the clinic) |
|---|---|
| Implant replacement or removal only (capsule kept) | When you only want replacement or removal with no other problem — helps prevent sudden adhesion, keep the contour and reduce tissue damage |
| Capsulotomy | Partial excision of the capsule when full removal is not needed and only the shape needs improving |
| Total capsulectomy | When rupture, capsular contracture, seroma or calcification requires full removal: the implant is taken out first, then the whole capsule |
| Combined lift | Periareolar, vertical or inverted-T incisions when sagging is a concern following removal |
Surgery information (per the revision sub-pages)
- Operation time: about 2 hours
- Anesthesia: general anesthesia
- Hospital stay: same-day discharge (same-day discharge or 1 night for capsular contracture and armpit-incision revision)
- Stitch removal: none
- Recovery period (as stated by the clinic): one week
- Incision: the previous armpit (about 2–4 cm) or breast-fold (about 3–4 cm) incision is reused
Diagnosis
Step 1 checks breast proportions and sets a suitable size in consultation; step 2 identifies internal problems with high-resolution ultrasound, which decides whether the capsule is removed partly or entirely.

Risks and side effects
- Revision is also implant surgery, so bleeding or hematoma, infection with inflammation and excessive fibrosis can occur again.
- The clinic states that breast revision is more difficult than the first operation.
- Removing implants can leave the breast sagging, so a lift is considered at the same time.
- Keloid-type scars can form; they are treated with scar injections.
Results and recovery vary by individual. Possible complications are explained during consultation.

Frequently asked questions
Does capsular contracture always need revision?
The clinic does not recommend revision for grades 1–2, where the change is small, and states that grades 3–4 may need revision following diagnosis.
Is the whole capsule always removed?
No. When only the shape needs improving, a capsulotomy removes part of the capsule; when rupture, capsular contracture, seroma or calcification requires full removal, a total capsulectomy is performed. This is decided from ultrasound and other test results.
Does revision always require a breast-fold incision?
The clinic states that if the implant was originally inserted through the armpit crease, revision can be done through the armpit crease; if through the breast fold, through the breast fold.
Can revision be done with Motiva Preservé?
The clinic does not recommend Preservé for revision patients, because the existing capsule must be corrected and a revision technique is more appropriate.
How much does revision cost?
The fee schedule (as of 2026-06-15) lists breast revision at KRW 10M–33M, VAT not included, varying with consultation and condition.
